FINANCIAL AND REFUND POLICY

       *Currently we are out of network with all insurance providers for all services except mental health and payment is required up front for all visits as stated below. We can provide you with a Superbill to submit to your insurance company, but we can not make any guarantees of reimbursement.*

Primary Care Services:

Self pay primary care (hypertension management, cholesterol, wellness exams, etc.) patient visits are $100.00. Labs are not included in self pay prices, but we do offer discounted labs if paid at the time of service. Please note, a lab collection and processing fee of $30.00 will be due at the time of service if you elect to use insurance for any lab specimen including, but not limited to, blood work, pap smear, swabs, urine. Procedures are not included in the visit price and are additional. Please inquire for your specific needs. We do offer direct primary care packages as well. Please note if using Impact Health Sharing services and you do not prepay for your visit, the associated visit fee is billed as $169.00.

 

Mental Health Services:
• We do accept most major insurances for mental health services. However, your benefits and eligibility may preclude coverage and reimbursement for services. Please make sure you are in good standing with your insurer before scheduling or attending an appointment. You will be responsible for your copay at the time of service. A card on file will be kept for any fees not reimbursed by insurance.
• Self pay mental health (counseling, medication management, etc.) patient visits are $225.00 for the initial intake and $150 for subsequent visits. Labs are not included in self pay prices, but we do offer discounted labs if paid at the time of service. Please note, a lab collection and processing fee of $30.00 will be due at the time of service if you elect to use insurance for any lab specimen including, but not limited to, blood work, pap smear, swabs, urine. Procedures are not included in the visit price and are additional. Please inquire for your specific needs. We do offer direct concierge mental health care packages as well.
• Direct mental health care memberships are available and billed on a monthly basis. This includes office visits, wellness exams, sick visits, and minor procedures. Lab work and medications will be at a reduced cost for membership plans. You will be billed for two months up front and then monthly afterwards. *Please note direct mental health care memberships are not a form of insurance and are only valid at Beyond Wellness Total Care, PLLC. This does not cover any specialist visits, emergency room visits, surgeries, or any associated hospital costs* Children (up to 18 years) associated with an adult membership are $150.00/monthly.  Adults 18 and up are $225.00/monthly. *

Direct primary care memberships are available and billed on a monthly basis. This includes office visits, wellness exams, sick visits, and minor procedures. Lab work and medications will be at a reduced cost for membership plans. You will be billed for two months up front and then monthly afterwards. *Please note direct primary care memberships are not a form of insurance and are only valid at Beyond Wellness Total Care, PLLC. This does not cover any specialist visits, emergency room visits, surgeries, or any associated hospital costs*

• Children (up to 18 years) associated with an adult membership are $50.00/monthly.

• Adults 18-30 are $125.00/monthly.

• Adults 31 and up are $150.00/monthly.

• 2 adults (any age) and 2 children $350.00/monthly (additional children under 18 are $25.00/monthly). Children under 18 are not eligible without a caregiver also having an active membership.

Self Pay Sick Visit Price Plans – Self pay sick visit price plans are a discounted visit price that includes your visit/examination by our health care provider, a procedure if necessary, certain laboratory testing, and a follow up visit for the same complaint if needed within 10 days. All labs will not be automatically complete and will only be administered if necessary. Please note that sick visits without testing are a flat $100.00 and do not include a follow up visit.

• Tier 1 $169.00 – Includes your initial visit, follow up within 10 days if necessary, and in clinic laboratory testing limited to urinalysis dipstick, urine pregnancy test, influenza testing, rapid strep test, and mononucleosis spot testing.

• Tier 2 $199.00 – Includes your initial visit, follow up within 10 days if necessary, and tier 1 laboratory testing plus Covid 19 rapid test if necessary.

• Tier 3 $249.00 – Includes your initial visit, follow up within 10 days if necessary, and one procedure deemed necessary, limited to incision and drainage, laceration repair, or splinting for fractures.

Medical Weight Loss:

• Our medical weight loss program is offered as a membership and is self pay only. All initial weight loss visits will cost $250.00, which will include the initial consultation and physical exam, measuring body composition with our handheld ultrasound machine, initial comprehensive lab panel, and a Vitamin B12 injection (if appropriate). If it is determined that you are a candidate for our weight loss program, we will determine if an oral or injectable regimen is the best treatment for you. The treatment regimens are billed monthly as a membership. There is not a guarantee that you will be a candidate for our weight loss program just by completing the initial consultation.

• The monthly membership fee includes repeat lab panel when deemed necessary or at 3 months, medication sent directly to you from a compounding pharmacy, and monthly follow up visits with one of our providers. Oral regimens will be $200.00 monthly and Semaglutide injectable regimens will be $400.00 monthly (up to 2mg dosing). Tirzepatide injectable regimens will be $500.00 monthly (up to 7.5mg). Tirzepatide regimens of 10mg monthly will be $600.00. Dosages above 10mg for Tirzepatide will be an additional cost and will be discussed prior to increasing. Most weight loss patients will not require above 7.5mg for Tirzepatide.

Women's Health:

• Women's health and hormone program is offered as a membership and is self pay only. All initial visits (not contraception) include your initial consultation and physical exam and a comprehensive hormone lab panel. The one time fee is $250.00. If hormone replacement therapy is determined to be appropriate for you, you would sign up for our monthly membership. There is not a guarantee that you will be a candidate for our women's health membership just by doing your initial consultation.

• Our women's health and hormone replacement therapy is offered as a membership and the monthly fee includes follow up labs when deemed necessary or at 3 months, medication sent to you directly from a compounding pharmacy, and monthly follow up appointments with one of our providers. The monthly cost is $175.00. If you use an alternative treatment regimen, the additional fee will be discussed prior to ordering your medication. If you are a candidate for testosterone therapy as well, the additional monthly fee is $40.00.

Men's Health:

• Men's health and testosterone program is offered as a membership and is self pay only. All initial visits include your initial consultation and physical exam and a comprehensive hormone lab panel. The one time fee is $250.00. If testosterone replacement therapy is determined to be appropriate for you, you would sign up for our monthly membership. There is not a guarantee that you will be a candidate for our men's health membership just by doing your initial consultation.

• Our men's health and testosterone therapy is offered as a membership and the monthly fee includes follow up labs when deemed necessary or at 3 months, medication sent directly to you from a compounding pharmacy for self administration, and monthly follow up appointments with one of our providers. The monthly cost is $175.00. If you use an alternative treatment regimen, the additional fee will be discussed prior to ordering your medication.

Empower RF:

• V Tone is sold as a package of 6 treatments for $800.00 or individual treatments for $275.00 each. Treatments can be performed every 1-3 weeks.

• Forma V is sold as a package of 3 treatments for $800.00 or individual treatments for $300.00 each.. Treatments can be performed every 2-4 weeks. 

• Empower Wellness Package is a combination of treatments with V Tone and Forma V. It is sold as a package of 3 treatments including both services for $1200.00.

Morpheus 8:

• Morpheus 8 is sold as a package of 3 treatments, but can be purchased individually as follows. Treatments are typically performed every 4 weeks.

  • Full Face for $1800.00 for a package of 3 treatments or $700.00 for individual treatments.​

  • Neck for $1600.00 for a package of 3 treatments or $600.00 for individual treatments.​

  • Face and Neck for $2600.00 for a package of 3 treatments or $950.00 for individual treatments.

  • Chest for $1500.oo for a package of 3 treatments or $600.00 for individual treatments.

  • Hyperhidrosis for $1600.oo for a package of 3 treatments or $600.00 for individual treatments.

  • Prime Around the Eyes for $1000.oo for a package of 3 treatments or $400.00 for individual treatments.

  • Prime Around the Mouth for $1000.oo for a package of 3 treatments or $400.00 for individual treatments.

  • Prime Around the Eyes and Mouth for $1200.00 for a package of 3 treatments or $450.00 for individual treatments.

• Morpheus 8 Body

  • Small area (upper arms, knees) for $2100.oo for a package of 3 treatments for one area or $850.00 for individual treatments. ​

  • Large area (abdomen, back of thighs, front of thighs, buttocks) for $2700.oo for a package of 3 treatments for one area or $1000.00 for individual treatments.

  • Add on an area to the package for additional $600 or add on to individual treatments for $250.00.

Empower Tone:

• Tone is sold as a package of 6 treatments for only $700.00 or individual treatments for $175.00 each. Treatments can be performed every 1-4 weeks.

Card on File: We require a card on file agreement for all patients. If you are on a monthly membership, your card on file will be used and charged on a recurrent basis. If you miss an appointment, your card on file will be charged a $50.00 fee as described below.

Patient Payment: The patient portion due for services rendered is expected at the time of service and if you are on a monthly membership, your card on file will be used for your recurrent payment. Payment must be received to receive prescriptions. Should an overpayment be made, the difference will be refunded. We accept all major credit cards, cash, venmo, zelle, and Cherry Payment Plans.

No Shows/Missed Appointments: We request notice to cancel or reschedule an appointment of at least 24 hours (1 business day) prior to the appointment. We have blocked off time for your appointment and if you are unable to attend, this time could be given to other patients who also need care. If appropriate notice is not given, a non-refundable charge of $50.00 will be assessed to the patient’s account/card on file. If a patient no shows to an appointment, the next scheduled appointment will need to be paid for in full prior to the appointment/at the time of scheduling and this fee is non-refundable.

Refunds for Treatment: If a patient decides to discontinue treatment after it has been started or medication has been ordered, a refund can not be given. No refunds will be given for services already rendered or for any membership fees/payments already collected. 

​

Collections: On occasion, after repeated attempts to collect a balance due, we may need to turn an account over to a collections agency. If this should occur, it is agreed that the financially responsible party shall pay all finance charges, collection costs, attorney fees, and any other costs that may be incurred to enforce collection of any outstanding balances.
